| I hereby authorize Camp Gladiator, Inc, a Texas corporation, to automatically charge my credit card, debit card or bank account in the amount of $69.00 per camp on the Tuesday of the off-week for six (6) consecutive camps. Spouse and friend referral discounts will be applied where necessary. I understand that Camp Gladiator, Inc. is released from any other obligation to furnish me with any further notices of this payment agreement. I understand that I cannot cancel the following as I am receiving a significant reduction in the normal charges for Camp Gladiator, Inc. fitness camps. I understand that I am liable for the complete balance agreed upon with this contract. I understand that after the six (6) camp payment term, that my account will continue to be automatically drafted $69.00 per camp, on a per camp basis for the same amount, unless I send written notice of cancellation via mail or fax fifteen (15) days prior to the payment date to Camp Gladiator, Inc.
